Please print and fill out a new Time and Talent sheet. These can be turned in at the church office or simply place in the offering plate. Thank you!
Youth Health form
For on and off site middle and high school youth ministries, we request a health and permission form that is good for one year. This will give leaders permission to treat the youth for any injuries that hopefully won't happen. It also gives the church permission to use photos in any publications we may put out. Please print one here and return it to the church office.
